Becoming an OMC Patient

Special Discount Program

OMC will offer to all eligible patients, to remove income as a barrier to care, a sliding fee discount based on income and family size and no other factor. The definition of income and family size will be based on the established current Federal Poverty Guidelines (FPG). Individuals and families with annual income below two hundred percent (200%) of the FPG are eligible for the sliding fee discount program.

The attached federal poverty scale outlines income limits by family size. To be considered for this program, you must complete the OMC Application for Discounted Health Services and you must be financially screened by Patient Services Staff in the clinics. Discounts are provided as part of the Outpatient Medical Center federal grant.
