Becoming a Patient

To become a Outpatient Medical Center (OMC) patient, please complete the Outpatient Medical Center Registration Form and the Medical History Questionnaire. For information on what documents to bring to the clinic see Items Needed to Register.

Our mission is to provide access to high quality primary care to any and all individuals in need.

Insurance Accepted

We welcome Medicaid, Medicare and private insurance. To find out if OMC accepts your insurance, please call billing services at (318) 357-3043 or email billing@outpatientmedical.org.

Special Discount Program

OMC will offer to all eligible patients, to remove income as a barrier to care, a sliding fee discount based on income and family size and no other factor. The definition of income and family size will be based on the established current Federal Poverty Guidelines (FPG). Individuals and families with annual income below two hundred percent (200%) of the FPG are eligible for the sliding fee discount program.

The attached federal poverty scale outlines income limits by family size. To be considered for this program, you must complete the OMC Application for Discounted Health Services and you must be financially screened by Patient Services Staff in the clinics. For information on what documents to bring to the clinic for financial screening see the Items Needed to Register and Apply for Discounts. Discounts are provided as part of the Outpatient Medical Center federal grant.
