Know what to expect at Outpatient Medical Centers (OMC). Our clinics provide compassionate and effective care to all patients, all ages, regardless an inability to pay. Translation services are available at request. OMC offers primary medical, preventive dental, and behavioral health services.

      The Tallulah site, Dental Dept., will begin seeing patients on Fridays, starting August 18, 2017 from 8am - 1pm
      The Tallulah site, Medical Dept., will begin seeing pediatric patients on Fridays, starting August 25, 2017 from 8am - 1pm


  2. What to bring with you:
    • Insurance card and photo ID
    • If you have no insurance, we offer a Sliding Fee Discount (SFD) program based on your income and household size as it relates to federal poverty guidelines. Enrolling in the SFD program will reduce your payment for services. To apply for the SFD program, bring the following with you to your next appointment:
      • Last two paycheck stubs, or;
      • Previous year tax return, or;
      • Unemployment statement

  3. When to Arrive?
    • If you are a current patient, please arrive 10 minutes prior to your appointment.
    • If you are a new patient, please arrive 30 minutes prior to your appointment.
    • If you are going to be late, please call and let us know, as we may reschedule your appointment after 20 minutes.

  4. Don't Have Insurance?
    • Call 318-352-9292 or email csmall@outpatientmedical.org, to schedule an appointment with our Outreach and Enrollment department. We can help you and your family understand your options, and apply for a health insurance plan that best works for you.
