Outreach and enrollment

Community outreach

Outpatient Medical Center's mission is to participate in activities designed to promote the general health of the community by promoting wellness without regard to physical or personal characteristics.

What are we about?

As a Community Health Center we have Certified Application Counselors on staff that are trained and educated in the different options for healthcare insurance and other types of State assistance. Certified Application Counselors (CAC) assist you on The Health Insurance Marketplace for insurance so you and your family can get and stay healthy.
CAC’s provide free year round assistance with your Health Insurance Marketplace application and/or state assistance programs including Medicaid and LaChip. We personally work with you to inform you about the best options for your budget, your health, and your future.

How do i apply?
